Conservatorship, Guardianship & Power of Attorney

How do I change the custodian for a UTMA account?

In order to change custodian for an UTMA, you’ll need to fill out a Transfer of Ownership Request form.

Follow the instructions provided in the form. Make sure you’ve done the following before mailing it in:

  • Obtain signatures from all new and existing account owners
  • Provide any required court documents

Once you’ve completed the form, mail it or fax it to our office using the address and fax numbers found on the form.

How do I change responsible individual, conservator, or guardian for a Coverdell Educational Savings Account?

In order to change the responsible individual, conservator, or guardian for a ESA, you’ll need to fill out a Transfer of Ownership Request form.

To initiate this change for a ESA, you must be the parent or legal guardian.

Follow the instructions provided in the form. Make sure you’ve done the following before mailing it in:

  • Obtain signatures from all new and existing account owners
  • Provide any required court documents

Once you’ve completed the form, mail it or fax it to our office using the address and fax numbers found on the form.

How do I change custodian, responsible individual, conservator or guardian for other accounts?

In order to change custodian, responsible individual, conservator or guardian, you’ll need to fill out a Transfer of Ownership Request form.

Do I need to provide Thrivent Mutual Funds with my court documents for guardianship or conservatorship? 

Yes. If the current custodian is still alive. If you were appointed by a court to make legal decisions for someone else, fax or mail a copy of the court documents to inform Thrivent Financial Investor Services, Inc.

How do I get a guardian or conservator appointed?

An attorney can help you file a petition with the court.

I am a power of attorney for someone. How do I get access to information about their account?

A copy of a power of attorney document must be faxed or mailed to Thrivent Financial Investor Services, Inc. before account information is available to you or you are able to make transactions on their behalf. Access will be available via phone. On-line access is not available at this time. It is your fiduciary responsibility to transact within the named powers of the power of attorney.
